To provide caseworkers with a sound understanding of the powers and duties of social service authorities to young people leaving care and how this interacts with the community care system for young people with disabilities

• Rights to leaving care provision
• Understanding  eligibility for leaving care provision
• Young people with disabilities – the transition between children and adult services
• The links between pathway and community care planning
• Common problem and how to tackle them
